• Complete this form in CAPITAL letters.
• This form must be completed in 2 copies by all qualified trained teachers from recognized Universities and Colleges.
• This application form must be addressed to the DEBS offices.
• Attach photocopies of Grade 12 Certificates, certified College/University certificates, and NRC.
• Attach a copy of the certificate of registration from the Teaching Council of Zambia.
• Persons with disabilities must attach a certificate from ZAPD.
• Applicants who are 45 years and above must attach proof of NAPSA contribution.
